Application Process

Please allow 10-14 days for the processing of paperwork.

• Please print and complete the forms below and leave with the school secretary. 
• The secretary will have the forms signed by the building principal.
• Your forms will then be sent to the District Office for the final signatures and mailed back to your home address.
• Take all documents to the Iowa DOT to obtain the school driving permit.
